ABSTRACT

OBJECTIVES: To explore best practices and challenges in providing school meals during COVID-19 in a low-income, predominantly Latino, urban-rural region. DESIGN: Semi-structured interviews with school district stakeholders and focus groups with parents were conducted to explore school meal provision during COVID-19 from June to August 2020. Data were coded and themes were identified to guide analysis. Community organisations were involved in all aspects of study design, recruitment, data collection and analysis. SETTING: Six school districts in California's San Joaquin Valley. PARTICIPANTS: School district stakeholders (n 11) included food service directors, school superintendents and community partners (e.g. funders, food cooperative). Focus groups (n 6) were comprised of parents (n 29) of children participating in school meal programmes. RESULTS: COVID-19-related challenges for districts included developing safe meal distribution systems, boosting low participation, covering COVID-19-related costs and staying informed of policy changes. Barriers for families included transportation difficulties, safety concerns and a lack of fresh foods. Innovative strategies to address obstacles included pandemic-electronic benefits transfer (EBT), bus-stop delivery, community pick-up locations, batched meals and leveraging partner resources. CONCLUSIONS: A focus on fresher, more appealing meals and greater communication between school officials and parents could boost participation. Districts that leveraged external partnerships were better equipped to provide meals during pandemic conditions. In addition, policies increasing access to fresh foods and capitalising on United States Department of Agriculture waivers could boost school meal participation. Finally, partnering with community organisations and acting upon parent feedback could improve school meal systems, and in combination with pandemic-EBT, address childhood food insecurity.

School-based nutrition programs are crucial to reducing food insecurity. The COVID-19 pandemic adversely impacted students' school meal participation. This study seeks to understand parent views of school meals during COVID-19 to inform efforts to improve participation in school meal programs. Photovoice methodology was used to explore parental perception of school meals in San Joaquin Valley, California, a region of predominately Latino farmworker communities. Parents in seven school districts photographed school meals for a one-week period during the pandemic and then participated in focus group discussions and small group interviews. Focus group discussions and small group interviews were transcribed, and data were analyzed using a team-based, theme-analysis approach. Three primary domains emerged: benefits of school meal distribution, meal quality and appeal, and perceived healthfulness. Parents perceived school meals as beneficial to addressing food insecurity. However, they noted that meals were unappealing, high in added sugar, and unhealthy, which led to discarded meals and decreased participation in the school meal program. The transition to grab-and-go style meals was an effective strategy for providing food to families during pandemic school closures, and school meals remain an important resource for families experiencing food insecurity. However, negative parental perceptions of the appeal and nutritional content of school meals may have decreased school meal participation and increased food waste that could persist beyond the pandemic.

ABSTRACT

The COVID-19 pandemic and the resulting control measures by the government impacted the travel behavior of Filipinos. With work trips being a major part of trips generated, this study aimed to investigate the effects of the pandemic on working Filipinos' mode choice. Data were collected from employed residents of Metro Manila using an online survey. About 48% of the respondents preferred using public transportation for work pre-pandemic. This decreased to 22% during the transport lockdown and increased slightly to 25% after the lockdown was lifted. Active transport share increased by 3% during the lockdown but went back down after it was lifted. Using multinomial logistic regression, the significant factors that influence the pre-pandemic respondents' mode choice were age, household income, travel cost, and vehicle ownership. During the lockdown, travel distance, travel time, and sex assigned at birth became significant also. After the lockdown, the significant factors reverted to that pre-pandemic.

ABSTRACT

Background/Objective: Studies have shown that Hispanics have low use of insulin pumps and continuous glucose monitors (CGM) when compared to Whites. (Agarwal, S, 2021, Apr). Racial disparities in technology use and diabetes outcomes persist regardless of insurance status. (Lipman, TH, 2021, Mar). Our aim was to obtain a better understanding of barriers that keep people from using advanced technology and prevent health disparities. Method(s): We completed semi-structured interviews and two focus groups with a sample group of Hispanic parents of type 1 diabetes (T1D) patients. We focused our interview on four main topics: sensor usage, pump vs. smart pen vs. MDI, training via video conferencing versus in-person, and suggestions to improve care. We compared the results to study what we did with Black parents. Result(s): We gathered data from 9 Hispanic mothers of T1D patients. The interview format was chosen by parents-22% individual interview, 11% email, and 66% focus group. All parents preferred interviews in Spanish. 100% reported using sensors regularly, 33% insulin pump, 55% smart pen, and one patient used neither. Key barriers to technology use were fear of doing something wrong at the beginning, difficulty with initial telemedicine training during COVID, and insurance problems, including language barriers and refills. Conclusion(s): Overall, when compared to our Black focus group, we received similar results. They are satisfied with the technology and feel it increases their understanding of diabetes. Insurance plays a limiting role. More studies are needed to increase understanding of our population and increase device use..

ABSTRACT

Currently, there are no evidence-based treatment options for long COVID-19, and it is known that SARS-CoV-2 can persist in part of the infected patients, especially those with immunosuppression. Since there is a robust secretion of SARS-CoV-2-specific highly-neutralizing IgA antibodies in breast milk, and because this immunoglobulin plays an essential role against respiratory virus infection in mucosa cells, being, in addition, more potent in neutralizing SARS-CoV-2 than IgG, here we report the clinical course of an NFκB-deficient patient chronically infected with the SARS-CoV-2 Gamma variant, who, after a non-full effective treatment with plasma infusion, received breast milk from a vaccinated mother by oral route as treatment for COVID-19. After such treatment, the symptoms improved, and the patient was systematically tested negative for SARS-CoV-2. Thus, we hypothesize that IgA and IgG secreted antibodies present in breast milk could be useful to treat persistent SARS-CoV-2 infection in immunodeficient patients.

ABSTRACT

OBJECTIVE: To assess the association between the timing of surgery relative to the development of Covid-19 and the risks of postoperative complications. SUMMARY BACKGROUND DATA: It is unknown whether patients who recovered from Covid-19 and then underwent a major elective operation have an increased risk of developing postoperative complications. METHODS: The risk of postoperative complications for patients with Covid-19 undergoing 18 major types of elective operations in the Covid-19 Research Database was evaluated using multivariable logistic regression. Patients were grouped by time of surgery relative to SARS-CoV-2 infection; that is, surgery performed: (1) before January 1, 2020 ("pre-Covid-19"), (2) 0 to 4 weeks after SARS-CoV-2 infection ("peri-Covid-19"), (3) 4 to 8 weeks after infection ("early post-Covid-19"), and (4) ≥8 weeks after infection ("late post-Covid-19"). RESULTS: Of the 5479 patients who met study criteria, patients with peri-Covid-19 had an elevated risk of developing postoperative pneumonia [adjusted odds ratio (aOR), 6.46; 95% confidence interval (CI): 4.06-10.27], respiratory failure (aOR, 3.36; 95% CI: 2.22-5.10), pulmonary embolism (aOR, 2.73; 95% CI: 1.35-5.53), and sepsis (aOR, 3.67; 95% CI: 2.18-6.16) when compared to pre-Covid-19 patients. Early post-Covid-19 patients had an increased risk of developing postoperative pneumonia when compared to pre-Covid-19 patients (aOR, 2.44; 95% CI: 1.20-4.96). Late post-Covid-19 patients did not have an increased risk of postoperative complications when compared to pre-Covid-19 patients. CONCLUSIONS: Major, elective surgery 0 to 4 weeks after SARS-CoV-2 infection is associated with an increased risk of postoperative complications. Surgery performed 4 to 8 weeks after SARS-CoV-2 infection is still associated with an increased risk of postoperative pneumonia, whereas surgery 8 weeks after Covid-19 diagnosis is not associated with increased complications.

ABSTRACT

BACKGROUND: The coronavirus disease 2019 pandemic necessitated an abrupt transition to exclusive virtual interviewing for maternal-fetal medicine fellowship programs. OBJECTIVE: This study aimed to assess the maternal-fetal medicine fellowship program directors' approaches to exclusive virtual interviews and to obtain program director feedback on the virtual interview experience to guide future interview cycles. STUDY DESIGN: A novel cross-sectional online survey was distributed through the Society for Maternal-Fetal Medicine to program directors after the completion of the interview season, but before the results of the National Resident Matching Program on October 14, 2020. Survey data were collected anonymously and managed using secure Research Electronic Data Capture electronic data capture tools. RESULTS: Overall 71 of 89 program directors (80%) responded. All respondents completed their 2020 interviews 100% virtually. Nearly half of program directors (33 of 68, 49%) interviewed more candidates in 2020 than in 2019. Of those who interviewed more candidates in 2020, the mean number of additional candidates per fellowship position was 5.8 (standard deviation, ±3.8). Almost all program directors reported no (35 of 71, 49%) or minimal (34 of 71, 48%) negative impact of technical difficulties on their virtual interview processes. Most programs structured their interview to a half day (4 hours) or less for the candidates. Many programs were able to adapt their supplemental interview materials and events for the candidates into a virtual format, including a virtual social event hosted by 31 of 71 programs (44%). The virtual social event was most commonly casual and led by current fellows. Ultimately, all program directors reported that the virtual interview experience was as expected or better than expected. However, most program directors felt less able to provide candidates with a comprehensive and accurate representation of their program on a virtual platform compared with their previous in-person experiences (46 of 71 [65%] reported minimally, moderately, or significantly less than in-person). In addition, most program directors felt their ability to get to know candidates and assess their "fit" with the program was less than previous in-person years (44 of 71 [62%] reported minimally, moderately, or significantly less than in-person). In a hypothetical future year without any public health concerns, there were 23 of 71 respondents (32%) who prefer exclusive in-person interviews, 24 of 71 (34%) who prefer exclusive virtual interviews, and 24 of 71 (34%) who prefer a hybrid of virtual and in-person interviews. CONCLUSION: The virtual interview experience was better than expected for most program directors. However, most program directors felt less able to present their programs and assess the candidates on a virtual platform compared with previous in-person experiences. Despite this, most program directors are interested in at least a component of virtual interviewing in future years. Future efforts are needed to refine the virtual interview process to optimize the experience for program directors and candidates.
